Job Description:

  1.  PATIENT CARE MANAGEMENT
  2. Make sure in the beginning of the scheduled shift that staff of the shift has arrived. If some scheduled staff did not turn up, review the available staff & their workload and re-allocate (staff & additional tasks) from low acuity areas to high acuity areas for the effective utilization of present staff.
  3. If it is hard to manage patient care activities, ask for extension of duty of previous staff during rush hours and concerns must be handed over to the morning shift to carry forward. No staff would do double duty or stay late for any meeting without prior approval.
  4. Document double duty or late stay arrangements in the log book as well as on area white boards in all shifts.
  5. Make sure staff allocation of all areas is completed in the log book as well as on areas white boards in all shifts, in accordance with patients’ acuity, including special assignments & staff break timing to avoid inconvenience.
  6. Carry out quick round to evaluate overall patient care, resources required, their availability and pay swift attention to urgent ones.
  7. Coordinate with previous shift supervisor to inquire about last 24 hours report (concerns/issues/events related to staff, patients & attendants) and handover priority bases.
  8. Conduct daily 10 minutes morning briefing with staff to discuss concerns about (staffing, patient care, incidents, complaints, appreciation or any other concern).
  9. Ensure staff compliance with duty hours (uniforms/attire, attendance & punctuality, hospital cards).
  10. Make sure healthcare workers behave professionally with patients/attendants and co-workers with the reflection of Indus Code of Ethics.
  11. Ensure wards are ready for physicians’ round including patients/beds and surroundings.
  12. Make sure bed-to-bed patients’ handover is in practice in all shifts and documentation in patient record.
  13. Join handover on regular basis in duty shift or designate to next level if you are busy in another task.
  14. Make sure designated ward/area inventory/crash cart/stock is physically verified against checklist & marked areas of improvement and ensure corrective/preventive actions.
  15. Ensure assigned staff is aware of patients’ care needs (hygiene care & change of clean hospital dress, bedding, etc.).
  16. Coordinate with consultants/medical officers to discuss patient care, personnel issues & resolve collaboratively.
  17. Ensure availability of consumable supplies and record of consumption.
  18. Make sure staff enter, follow supplies & consume used supplies in HMIS.
  19. Hold weekly meeting with senior nursing team (NSSSs & TLs) to review staffing & nursing service outcomes.
  20. Liaise with CNI on daily/weekly basis to discuss in-service education needs, training & development activities.
  21. Keep the Nursing Management informed of any special needs.
  22. Conduct random rounds with staff to see their allocated patient care & overall management under supervision.
  23. B. ENSURING PATIENTS MONITORING, ASSESSMENT, CARE PLANS AND DOCUMENTATION IN PATIENTS RECORD
  24. Ensure completion & review of patients’ initial assessment, prioritization of care needs & completion of NCPs
  25. (Fall prevention, Cluster NCP and Cluster Obs/Gynae NCP)
  26. Ensure staff monitor & record fresh vital signs, height & weight of new admissions and documentation in patients’ record
  27. Ensure provision of hygiene care as per patients’ individual needs (Activity of Daily Living) of bed bound / activity restriction due to disease requirement (AM & PM care) & documentation
  28. Ensure each patient’s vital signs, height & weight checked and documented as per monitoring order / policy
  29. Ensure exact calculation of intake and output, drainage and documentation in patient record
  30. Ensure prescribed Investigations of the patient (Labs, Urine and stool specimens) are collected, labelled & dispatched to the Lab for early results
  31. Ensure Care of I/V line / cannula, CVCs / lines and drains and tubes is taken as per TIH guideline
  32. Ensure dressing of wounds is carried out following IC guideline
  33. Extend help in direct patient care, at times of staff shortage and in emergency situations
  34. C. MEDICATION MANAGEMENT
  35. Ensure staff is aware of medication management flow process (prescription, identification of drugs, dispensing, physical verification of dispensing & acknowledgement in HMIS, verification against rights of Drugs safety prior to preparation, labeling of prepared drug, administration after patient & drug safety identification and documentation in patient’s HMIS record)
  36. Ensure duty doctor is aware of Not Available (NA) medication process, its requisition & documentation in patient record about LP form & follow up with pharmacy
  37. Ensure timely dispensing and administration of medication in accordance with drug/patient safety rights & its documentation in patients’ HMIS record or in nurses’ event if module is not available in HMIS
  38. Ensure practice compliance with independent double check of High alert / LASA medications, blood & blood products transfusion prior to preparation and prior to administration at patient site or bed side
  39. Initiate incident report if drug is not as per order, take pictorial evidence and resend drug to pharmacy for change, so patient will get drug as early as possible
  40. Ensure renewal of medication for patients in IPD on daily basis
  41. D. MONITORING OF INVENTORY AND MAINTENANCE OF EQUIPMENT
  42. Ensure designated Ward/area inventory is physically verified against checklist & marked in each shift
  43. Ensure staff training from Bio-Medical about equipment operation, cleaning and functionality
  44. Ensure that missing inventory is reported to accountable & HOD decide official action
  45. Ensure cleaning and disinfection of equipment as per IC guideline and their safety reporting
  46. E. MONITORING OF STAFF PERFORMANCE AND AREAS OF IMPROVEMENT
  47. Evaluate staff teaching needs & organize Ward Teaching Program accordingly
  48. Participate in clinical teaching and staff development programs
  49. Counsel and guide all nursing staff as per their individual needs
  50. Equipment & Inventory Management Responsibilities
  51. Ensure staff training from Bio-Medical about equipment operation, cleaning and functionality
  52. Make sure that missing inventory is reported to accountable & HOD decide official reporting
  53. Ensure cleaning and disinfection of equipment as per IC guideline and HOD decide official reporting
  54. Ensure out of order/broken equipment reported to biomedical for repair, label “out of order” & followed with Bio-Medical
  55. F. QUALITY ASSURANCE PERSPECTIVE
  56. Ensure compliance with the IHHN policies and procedures in all shifts
  57. Complete tasks assigned by the Manager / Deputy Manager in all shifts (Audits, indicators monitoring & complaints/incidents investigation etc.)
  58. Report and encourage staff to report incidents after consulting with shift supervisors/senior if necessary
  59. Facilitate / escort special visitors on the hospital round
  60. Participate in department, Nursing Division and QMD CQI KPI
  61. Take part in 24hrs, monthly and quarterly reports
  62. Prepare assigned areas for audits (Nursing, QIPS, IHHN internal/external, & PHC etc.)
  63. Ability to initiate incident / sentinel events, investigate including and timely completion in collaboration with QAD
  64. G. HUMAN RESOURCE MANAGEMENT
  65. Make sure that new joiners orientation objective is discussed with staff to complete within probationary period.
  66. Ensure smooth running of department orientation of new joiner, their clinical placement with new joiner orientation checklist, discuss monthly progress of objective with individual staff, indicating areas of improvement and keep record.
  67. Ensure finalization of timely duty roster a week earlier of schedule month, enter in HMIS, display on the notice board for the early view of staff and keep a signed copy for each category under reporting for daily review.
  68. Ensure staffing budget as per agreed nurse patient ratio with HR, staff hiring in collaboration with HR.
  69. Ensure timely probationary confirmation with formal meeting and constructive feedback.
  70. Review biannual individual in person objective of staff to discuss areas of improvement with anecdotal record to use in summative & formative performance evidences to avoid disagreement.
  71. Review annual performance with staff.
  72. Demonstrate fair judgement while discussing performance with each staff in person against objective, share strengths & weaknesses.
  73. Communicate timely resignation/absenteeism/nonprofessional behavior before sharing with HRD reviewer.
  74. Ensure personnel growth, renewal of competencies & certifications of staff to HRD.
